Thank you for your interest in having your chickens processed at Lowbank Farms. Before you proceed please note the following:

Form 300 – Small Flock Registration: Small-flock farmers must provide their Form 300 registration information when bringing chickens to Lowbank Farms for processing. Farmers who purchase chicks through a hatchery or broker-dealer may receive their Form 300 information at the time of purchase. Farmers who hatch their own broilers can obtain a Family Food Program registration number and Form 300 through CFO (289-288-4231).

Form 36 – all Quota Holders must submit a Form 36 and Farm Number. Contact your CFO representative if you do not have this form.

Transport Details Form - MUST be completed at the time of poultry drop-off.
